About the Role
We are seeking a dedicated and compassionate Support Coordinator to join our Care Services team on a permanent part-time basis.
In this role, you will support NDIS Participants to understand, implement, and maximise their NDIS plans, ensuring they are connected with the right mix of informal, mainstream, and funded supports to achieve their goals.
You will work within a competitive service environment where quality outcomes, participant satisfaction, and strong professional relationships are essential to success.
This role suits someone who is organised, person-centred, and confident navigating complex service systems to advocate for and empower participants.
Key Responsibilities
- Support NDIS Participants to identify goals and develop strategies within a person-centred framework
- Assist participants to implement their NDIS plans and maximise value for money from supports
- Connect participants with informal, mainstream, and funded supports (including allied health providers and community services)
- Build and maintain strong, professional relationships with participants, families, and service providers
- Advocate for participants and facilitate access to appropriate community and mainstream services
- Promote independence, empowerment, social inclusion, and resilience
- Liaise with carers, families, NDIA, LACs, and external service providers
- Assist in problem solving and responding to emerging participant needs
- Maintain accurate case notes, documentation, and reporting within required timeframes
- Work in accordance with NDIS guidelines, relevant legislation, and Thrive policies and procedures
- Contribute to service improvement, workflow enhancements, and team collaboration
About You
- Experience or strong understanding of NDIS Support Coordination and its role in building participant capacity
- Knowledge of disability, mental health, or community service systems (including mainstream services such as health, education, housing, or justice)
- Understanding of the episodic nature of mental illness and its impact on functioning (highly regarded)
- Strong communication skills, both written and verbal
- Ability to build trust-based, professional relationships with participants and stakeholders
- Confident using Microsoft Word, Excel, and online client management systems
- Ability to manage competing priorities in a dynamic service environment
- Demonstrated ability to work independently and prioritise workload effectively
- Empathetic, respectful, and committed to person-centred practice
Essential Requirements
- Current Working With Children Check
- Current NSW Driver’s Licence
- NDIS Worker Screening Check (or willingness to obtain)
- Valid First Aid / CPR
- Must have the right to work in Australia
